The partnership inquiry form collects your name, business email, organization, organization type, business goal, optional clinical starting point, and any optional states, patient-volume range, or message you provide. It also records your consent, submission time, and a reference number.

This site is intended for business inquiries. Please do not submit patient records, diagnoses, insurance member IDs, or other health information.

How inquiry information is used

Inquiry information is stored to review business fit, respond to your request, and discuss a possible partnership. It may be accessed by personnel and service providers involved in operating the site and handling the inquiry. Site hosting providers may process technical information needed to deliver and protect the website.

Website technology

This website uses hosting and database services to display pages and store submitted inquiries. The readiness explorer works in your browser and does not submit your answers until you choose to send an inquiry. We have not added advertising pixels or behavioral advertising trackers. Hosted access controls may use essential authentication technology.

Fonts may be loaded from Google Fonts, which receives ordinary web request information when those resources load. If you follow an external link or send an email, the relevant provider’s privacy practices apply.

Your information and choices

To ask about your submitted information, request a correction or deletion, or withdraw permission for follow-up, email contact@telehealthus.com with “PayorPartners privacy” in the subject. We may need to verify your request before acting on it. Information may be retained as reasonably needed for the inquiry, business records, or applicable obligations.
